The project proposed seeks to explore the models of community nursing care within three integrated joint boards in NHS Grampian. With the introduction of Health and Social Care Integration, community services for patients and their families who have challenging care needs and require expert help and support at home is undergoing significant change. At present, there is a dearth of research and information nationally and internationally as to what models of care best meet the needs of patients and carers. This project will explore different models of care in the IJBs across Grampian
